How to reduce creatinine levels and keep kidneys healthy ?

My friend who is about 25 years is suffering from kidney damage since 2 years. He is in stage 3 kidney damage doctors are recommending for kidney transplant in future.Isn't there any alternative option to manage kidney health? He also used to have lower urine flow but after using medications he is having better urine flow than earlier.Recently he observed foamy urine which is concerning. He wanted to maintain kidney health and creatinine levels please help us and recommend which diet he should have.
