Hello
Trouble breathing and a heavy-headed feeling starting after a punch to the face should not be ignored. While it could be related to pain, swelling, anxiety, or a nasal injury, it can also indicate a more significant injury affecting your nose, sinuses, airway, or head.
If your breathing difficulty has lasted more than 24 hours and you also have dizziness/lightheadedness and facial swelling, you should be evaluated by a doctor as soon as possible, preferably today. It’s not possible to tell remotely whether low oxygen is the cause. A doctor may need to examine your nose, face, and neurological status, and sometimes imaging is required.
If you do not see a doctor, a potentially serious injury could be missed, such as a nasal fracture, facial fracture, concussion, or another head injury. Most punches do not cause life-threatening problems, but persistent breathing difficulty after facial trauma warrants assessment.
Seek emergency care immediately if you develop worsening shortness of breath, severe headache, repeated vomiting, increasing drowsiness, confusion, vision changes, weakness, loss of consciousness, chest pain, or worsening facial swelling.
Given that the breathing problem has persisted for over a day after the injury, I would recommend getting in person medical attention today rather than waiting.

The patient’s symptoms of mild breathing difficulty, facial swelling/bruising, dizziness, and a heavy-headed feeling occurring after a punch to the face raise concern for a facial injury and possible mild concussion. While the symptoms may be due to swelling and trauma-related effects, persistent breathing difficulty for more than 24 hours should not be ignored. If left unevaluated, an underlying nasal or facial bone injury, worsening swelling, or a head injury could be missed. The patient is advised to seek prompt medical assessment, especially if symptoms worsen or if severe headache, increasing shortness of breath, vomiting, confusion, vision changes, or loss of consciousness develop.
