Hello, Based on your description, your 11-month-old son has had 2 episodes of diarrhea, 1 episode of vomiting, and is now crying, especially when his right ear is touched. Since he also has cough/cold symptoms, this raises the possibility of acute otitis media (middle ear infection), which is common in young children after a viral upper respiratory infection. The diarrhea and vomiting may be due to a viral illness and may or may not be related to the ear infection.

For now: • Continue offering small, frequent feeds and ORS to prevent dehydration. • Monitor his urine output and activity level. • If he is crying because of pain, a pain reliever is appropriate.

Prescription/Advice: • Syrup Paracetamol 15 mg/kg per dose every 6 hours as needed for pain or fever (do not exceed the recommended daily dose). • Continue ORS in small frequent sips after each loose stool. • Continue breastfeeding/formula feeds as tolerated. • Arrange an examination by a pediatrician today, as his ear needs to be examined with an otoscope. Antibiotics should only be started if an ear infection is confirmed. • Do not put any ear drops into the ear unless advised by a doctor.

Please seek urgent medical care immediately if he develops: • High fever • Persistent vomiting or inability to drink • Very few wet diapers • Ear discharge • Excessive drowsiness, difficulty waking, or inconsolable crying

He should be examined by a pediatrician today so the ear can be checked with an otoscope. Antibiotics are only needed if an ear infection is confirmed.

In the meantime, continue offering small, frequent feeds (breast milk, formula, or oral rehydration solution) to prevent dehydration from the diarrhea and vomiting. For pain, you may give paracetamol (acetaminophen) 10–15 mg/kg every 4–6 hours as needed (maximum 4 doses in 24 hours), provided there is no allergy or contraindication. Avoid putting any drops or oil into the ear unless prescribed.

Seek urgent medical care immediately if he becomes difficult to wake, has repeated vomiting, develops a fever, has swelling or redness behind the ear, pus or blood from the ear, shows signs of dehydration (very few wet diapers, dry mouth, no tears), or if the crying becomes inconsolable despite pain relief.

Hello dear See as per clinical history it seems Otitis media Fortunately the pedestician has already prescribed collimex medication for fever and diarrhoea. But it seems emergency so i suggest you to please get in person consultation with pediatrician in person for better clarity Regards
