First, it’s important to address the elevated IgE levels and eczema, which suggest an allergic component is contributing to her symptoms. High IgE levels can often be associated with atopic conditions, including eczema. However, the delayed wound healing and hyperpigmentation (the black marks left on wounds) may or may not be related, so it’s crucial to evaluate those separately. Despite the normal CBC, blood sugar, and other tests, it would be prudent to consider further dermatological evaluation. This includes a patch test to rule out contact dermatitis or other skin conditions that might contribute to her eczema and delayed healing.

For treatment, a multi-faceted approach often yields the best results for eczema and associated symptoms. Topical corticosteroids can help reduce inflammation and itchiness related to eczema. If the wound healing is poor despite these measures, consider moisturizing the skin with emollients as dry skin can hinder healing. They should be applied multiple times daily or even more frequently if the skin is noticeably dry. Have her avoid known irritants—certain soaps, detergents, or foods exacerbating her symptoms from her environment if these have been identified previously.

A referral to an allergist or immunologist might be necessary for further testing, considering her high IgE level, to see if more rare or overlooked allergens are affecting her. Additionally, if diet could be contributing to her symptoms, a dietitian might provide guidance without relying solely on food allergy tests.

Her lifestyle factors, such as stress, should also be reviewed as they can impact eczema severity and healing times. If standard treatments aren’t effective, phototherapy and systemic treatments like biologics can also be considered but require more careful risk management and planning.

Lastly, although the doctor mentioned food allergy tests might be unreliable, maintaining a food and symptom diary could provide insights into any correlations, especially with her skin condition. It is essential to follow up regularly with healthcare providers to monitor progress and adjust any treatments as needed based on response and any emerging symptoms. If there are no significant improvements or the symptoms worsen, further specialist referrals may be key in getting a comprehensive treatment strategy.
