Doctors' responses

Dr. Perambalur Ayyadurai Rohith
I am a general physician with more than 10 yr of clinical experiance, and in this time I worked with patients from all age groups, from young kids to elderly with multiple chronic issues. My practice has been wide, but I gradually developed deeper intrest in diabetology. I spend much of my day focusing on prevention, early diagnosis and management of diabetes, using lifestyle modification, medical therapy and regular monitoring. Many patients come worried about complications, and I try to explain things in simple language, whether it is diet, excercise, or understanding lab reports, so they dont feel lost. I also conduct detailed diagnostic evaluation and use evidence based protocols to make sure treatment is reliable and updated, even if sometimes I double check myself when results dont match the clinical picture. Apart from regular OPD practice, I gained strong experiance in occupational health. Over years I worked with multiple companies handling pre employment checks, annual medical exams, workplace wellness programs, and ensuring compliance with industrial health and safety standards. It is diffrent from hospital practice, but equally important, because healthy workers mean safe and productive workplace. I run medical surveillance programs and health awareness sessions in collaboration with corporates, and this also gave me exposure to preventive strategies on a large scale. For me, patient care is not just treatment but building trust. My career revolve around preventive medicine, ethical clinical practice, and continuous learning. I keep myself updated with modern medical protocols, but I also value listening to patient worries, since medicine is not only about lab values but also about how a person feels in daily life. I make mistakes in words sometimes, but in my work I try to be very precise. At end of day, my aim is to provide care that is accessible, evidence based and truly centered on patient well being.

1. Sciatica with L5–S1 disc bulge can cause radiating pain, numbness, and burning your symptoms fit this pattern.

2. Since pain has recurred despite Ayurvedic treatment, further evaluation with MRI spine is advisable.

3. Strict diabetic control is important because high sugars worsen nerve pain and healing.

4. Physiotherapy with specific exercises, posture correction, and avoiding heavy lifting are crucial in long-term relief.

5. If pain or numbness worsens, consult an orthopedician/spine specialist for medical or interventional options.

Your symptoms of low back pain and radiation to the right leg suggest sciatica likely due to an L5-S1 disc issue, which can cause pain, numbness, and burning sensations. Given your history of diabetes and the recurrent nature of these symptoms, it’s essential to consider both degenerative disc disease and diabetic neuropathy as contributing factors. The Ayurvedic treatments appear to have provided limited relief, so exploring other evidence-based options may be beneficial. First, consult a healthcare professional like a neurologist or orthopedist to assess the severity of the disc bulge with MRI imaging. This will determine if there’s nerve root compression requiring physical therapy or surgical consultation. Pain management includes analgesics like NSAIDs or, if needed, stronger medications prescribed by your doctor. Physical therapy exercises strengthen supporting muscles. Lifestyle adjustments such as weight management and ergonomic posture help long-term. For the diabetic neuropathy, maintaining optimal blood glucose levels is key. Your fasting and postprandial sugar levels suggest some room for improvement. Discuss possible medication adjustments or dietary changes with your endocrinologist. Supplementing with Vitamin D3 and Methylcobalamin seems appropriate, but include these in a comprehensive plan overseen by your doctor. If symptoms like weakness or loss of bladder control appear, seek urgent evaluation as these may signal more severe nerve issues. Continuing with a multidisciplinary approach, balancing medical interventions and self-care, will likely yield better results.
