Doctors' responses

Dr. Bharat Joshi
I’m a periodontist and academician with a strong clinical and teaching background. Over the last 4 years and 8 months, I’ve been actively involved in dental education, guiding students at multiple levels including dental hygienist, BDS, and MDS programs. Currently, I serve as a Reader at MMCDSR in Ambala, Haryana—a role that allows me to merge my academic passion with hands-on experience. Clinically, I’ve been practicing dentistry for the past 12 years. From routine procedures like scaling and root planing to more advanced cases involving grafts, biopsies, and implant surgeries. Honestly, I still find joy in doing a simple RCT when it’s needed. It’s not just about the procedure but making sure the patient feels comfortable and safe. Academically, I have 26 research publications to my credit. I’m on the editorial boards of the Archives of Dental Research and Journal of Dental Research and Oral Health, and I’ve spent a lot of time reviewing manuscripts—from case reports to meta-analyses and even book reviews. I was honored to receive the “Best Editor” award by Innovative Publications, and Athena Publications recognized me as an “excellent reviewer,” which honestly came as a bit of a surprise! In 2025, I had the opportunity to present a guest lecture in Italy on traumatic oral lesions. Sharing my work and learning from peers globally has been incredibly fulfilling. Outside academics and clinics, I’ve also worked in the pharmaceutical sector as a Drug Safety Associate for about 3 years, focusing on pharmacovigilance. That role really sharpened my attention to detail and deepened my understanding of drug interactions and adverse effects. My goal is to keep learning, and give every patient and student my absolute best.

The symptoms you’re describing - severe headaches, migraine-like symptoms, and scalp tenderness - could be related to a few different conditions. One possibility is that you are experiencing chronic migraines, which can include scalp or cutaneous allodynia (pain on the skin’s surface). This is where normally non-painful stimuli, like brushing your hair, become painful. Another potential cause could be occipital neuralgia, which involves irritation or injury to occipital nerves, leading to severe head pain and scalp sensitivity. It’s also possible that tension headaches, which can create pressure around the head and tenderness over the scalp, might be contributing to your symptoms.

Given the chronic nature and severity, it would be prudent to explore further diagnostic testing. A CT scan or MRI could help rule out other structural causes of headache, such as a mass or other pathology that might be influencing nerve sensitivity. It’s also essential to review your symptoms in detail with a neurologist or specialist in headaches, as they can assess whether your pain might be connected to any other underlying issues, including potential neurological conditions. Discussing preventive treatment options would be beneficial, as there are medications and lifestyle adjustments that can reduce the frequency and intensity of migraine attacks.

Meanwhile, try to track your triggers - anything from food to stress or lack of sleep can exacerbate headache patterns. Recognizing these could help in managing symptoms. Be mindful of the frequency and intensity of headaches, and any additional neurological symptoms, such as blurred vision or confusion, as these warrant immediate evaluation. Ultimately, a tailored and comprehensive approach involving both diagnostic tests and a deeper symptom evaluation can help clarify the root cause of your symptoms and lead to more effective treatment strategies.
