Gina smith

I am currently taking ativan 4 mg daily for anxiety and effexor 75 mg daily. Just cross tapered off lexapro with last dise taken a week ago. Have been taking effexor fir about a month. Still having periods of severe anxiety and feelings of disconnection. Should this be happening on my current regimen? Also waking during the night and morning with anxiety. What would you recommend

Doctors’ responses

It’s not uncommon to still experience anxiety during a medication transition, like you’re describing with the cross taper from Lexapro to Effexor and ongoing use of Ativan. Effexor (venlafaxine) usually takes several weeks to build up to a therapeutic level, especially when transitioning from another antidepressant. However, if you’re having severe anxiety and disconnection, these could be side effects or signs that your current regimen might need some adjustments. With Ativan (lorazepam) at 4 mg daily, which is a relatively higher dose, tolerance could reduce its effectiveness, or the medication could be contributing to feelings of disconnection or even worsening your anxiety. The waking anxiety might relate to the natural dip in medications levels overnight or potentially other issues like withdrawal from previous medication. It’s really important to consult your prescribing doctor here for an assessment. They can help evaluate whether your Effexor dosage needs adjusting or if any other issues might be influencing your symptoms. It’s also worth discussing non-drug strategies, like cognitive-behavioral therapy (CBT), that can be extremely beneficial alongside medication to manage anxiety. Night-time routines that focus on reducing stimulation and practicing relaxation techniques before bed might help alleviate waking anxiety. Be open with your healthcare provider about these symptoms so they can tailor a plan specific to your needs. Remember, while Effexor takes time to work, you shouldn’t feel worse during the process. Immediate evaluation by your doctor is advisable if anxiety becomes overwhelming or unmanageable.
