Given your friend’s situation with stage 3 kidney disease, it’s critical to take a proactive approach to manage his kidney health. Kidneys at this stage have a moderate reduction in function, and maintaining kidney health largely centers on slowing progression and managing symptoms. First, it’s vital to keep blood pressure under control since high blood pressure can further damage the kidneys. Aim for a target of below 130/80 mmHg if tolerated, and it’s often best to achieve this with medications as prescribed by a healthcare provider. Blood glucose control is equally important if your friend has diabetes. Diet plays an essential role, and he should focus on reducing sodium intake to no more than 2,300 mg per day, while opting for fresh, unprocessed foods. Protein intake needs to be moderated as high protein diets can stress the kidneys; generally, limiting protein intake to about 0.6 to 0.8 grams per kg of body weight per day helps. However, individual needs may vary, so consulting a dietitian knowledgeable in kidney disease could be beneficial. Staying hydrated is important, but excessive fluid intake should be avoided based on the physician’s advice regarding total fluid allowance. Monitoring and discussing phosphorus and potassium levels with a doctor is crucial as these can build up in the blood when the kidneys are not functioning properly. Foamy urine might be a sign of proteinuria, indicating protein in the urine, which often correlates with kidney problems. Make sure he raises this with his doctor for further evaluation. Avoiding NSAIDs and other nephrotoxic medications, and ensuring regular follow-ups with a nephrologist to adjust any treatments and monitor kidney function is necessary. Urgent medical attention is needed if he experiences symptoms like severe swelling, high blood pressure, or difficulties breathing. While transplantation is a potential option down the road, these lifestyle and dietary adjustments, along with medical management, can often stabilize function and delay progression.
