Doctors’ responses

Dr. Perambalur Ayyadurai Rohith
I am a general physician with more than 10 yr of clinical experiance, and in this time I worked with patients from all age groups, from young kids to elderly with multiple chronic issues. My practice has been wide, but I gradually developed deeper intrest in diabetology. I spend much of my day focusing on prevention, early diagnosis and management of diabetes, using lifestyle modification, medical therapy and regular monitoring. Many patients come worried about complications, and I try to explain things in simple language, whether it is diet, excercise, or understanding lab reports, so they dont feel lost. I also conduct detailed diagnostic evaluation and use evidence based protocols to make sure treatment is reliable and updated, even if sometimes I double check myself when results dont match the clinical picture. Apart from regular OPD practice, I gained strong experiance in occupational health. Over years I worked with multiple companies handling pre employment checks, annual medical exams, workplace wellness programs, and ensuring compliance with industrial health and safety standards. It is diffrent from hospital practice, but equally important, because healthy workers mean safe and productive workplace. I run medical surveillance programs and health awareness sessions in collaboration with corporates, and this also gave me exposure to preventive strategies on a large scale. For me, patient care is not just treatment but building trust. My career revolve around preventive medicine, ethical clinical practice, and continuous learning. I keep myself updated with modern medical protocols, but I also value listening to patient worries, since medicine is not only about lab values but also about how a person feels in daily life. I make mistakes in words sometimes, but in my work I try to be very precise. At end of day, my aim is to provide care that is accessible, evidence based and truly centered on patient well being.

1. Since your father is diabetic and has kidney and heart conditions, skin infections can spread faster and take longer to heal.

2. Poor blood circulation, high sugar levels, and anemia reduce the body’s ability to fight infection.

3. Keep the affected area clean and dry, avoid scratching, and use mild antiseptic soap.

4. He should urgently consult a doctor for proper antibiotics/antifungal medicines, as home care alone will not be enough.

5. Please keep his blood sugar, blood pressure, and heart medications strictly under control to help recovery.

I’m Dr. Rohith Ayyadurai, MBBS, Diabetologist & General Physician. Available for further guidance if needed.

Skin infections can be concerning, especially with underlying conditions like ischemic heart disease, hypertension, Type 2 diabetes mellitus, chronic kidney disease, and anemia, which can complicate the healing process and increase the risk of complications. Your father’s history of diabetes and CKD, in particular, can impair his immune response, making it important to address these skin infections promptly. The first step is to identify whether the infection is bacterial, fungal, or another type. If it’s bacterial, typically antibiotics may be necessary, but the choice of antibiotic should be guided by a doctor’s evaluation and possibly a culture test to pinpoint the specific bacterium involved. It’s also important for diabetes management to be optimized, as fluctuating blood sugar levels can impair healing. Regular blood glucose monitoring and managing his diet accordingly can help in controlling blood sugar. For hypertension and anemia, maintaining their management is crucial during this period too, as they can affect overall health status, including the skin’s ability to heal. Heart disease and SOB also warrant careful attention. Make sure to monitor for signs of worsening infection, like increased redness, swelling, or fever, which could necessitate urgent medical attention. In cases where the infection does not improve or worsens, a more in-depth assessment by a healthcare provider is crucial. Additionally, maintaining good hygiene practices, moisturizing the skin, and avoiding any irritants can further assist in recovery. Ensure any medications for these chronic conditions are taken consistently unless advised otherwise by a healthcare professional.
