Hi Tejas, Your symptoms suggest chronic or recurrent balanitis, which can occur due to infection (fungal or bacterial), irritation, or an underlying condition like diabetes or tight foreskin. Here is my advise-

1. Kindly visit a certified Dermatologist or a Family Physician for clear physical examination of your skin issue. Taking consultation like this might land you with wrong medicines that will do more harm than benefit. So, take a proper consult. Till then take Tab. Levocetirizne 5mg at night for itching.

2. Additional Measures: Keep the area clean and dry. Avoid retraction of foreskin if painful. Wear loose cotton underwear.

3. Get done these tests - CBC, Fasting Glucose, Swab/Culture from the lesion.

4. Long-Term Prevention: Maintain daily gentle hygiene. Avoid strong soaps, powders, and antiseptic liquids. Dry the area after urination or bathing.

It seems to be fungal balanitis.

Rx: Candid cream twice daily for 1 -2 weeks.

Clean gently twice daily with lukewarm water only (no soap on the glans). Avoid harsh soaps, antiseptics, or alcohol-based products. Dry gently after washing; don’t rub. Avoid sexual activity until it heals completely. Wear loose, breathable cotton underwear; change daily. Avoid irritants: scented lotions, condoms with lubricants or spermicides. If diabetic, check and control blood sugar -high sugar promotes fungal growth

It persists or not reducing ,please consult a urologist or physician or dermatologist in person.
