How to care itchiness and pimple on the scalp and near hairline? - #10745

Since two months, My scalp is very itchy and i have pimples on the scalp and hairline which causes more hairfall. I changed my shampoo which is a mild one now. I haven't tried any medication yet. I have tried ketoconazole shampoo once a week.

Like one hour I’m in resus managing a poisoned patient, next I’m guiding a batch of med students through triage protocols. It’s demanding, but it keeps me sharp. I try not to get stuck in patterns—I mean emergency medicine is always shifting. I stay updated with new guidelines, courses, journals—some of it sticks, some doesn’t haha. But that’s how you keep growing. My thing is blending strong protocols with some human stuff—listening better, watching closer, explaining things in a way people can actually get. And yeah, mentoring’s important to me. Watching younger docs or interns go from nervous to confident... feels good. I’m all for building that bridge between practice and teaching, science and instinct. Maybe I don’t always get it right—but I care, a lot. That counts right?

Your Symptoms:

🔍 Probable Diagnosis:

Your symptoms suggest seborrheic dermatitis or folliculitis (scalp inflammation due to excess oil, fungal growth, or bacterial infection), which are common causes of:

Scalp pimples

Itching

Hair fall due to inflamed follicles


✅ Treatment Plan:

1. Medicated Shampoo Routine:

Ketoconazole 2% shampoo (e.g., Nizoral): Use twice a week – leave on scalp for 5–10 minutes before rinsing.

On other days, use a mild sulfate-free shampoo (e.g., Sebamed, Baby shampoo, or Ducray Extra-Doux).

2. Topical Treatment (Apply directly to pimples & itchy areas):

Clindamycin gel 1% OR Nadifloxacin cream – apply twice daily on scalp pimples.

If severe: You may need a steroid+antibiotic lotion like Elocon lotion (Mometasone) or Clobetasol + Neomycin lotion, but only under doctor’s supervision.

3. Oral Medication (if not improving):

Doxycycline 100 mg once daily for 7–10 days (for scalp folliculitis) – needs a doctor’s prescription.

Antihistamine like Levocetirizine 5 mg at night for 7–10 days to reduce itching.

Avoid oily, fried, spicy food

Drink 2–3 liters of water daily

Add zinc-rich foods (pumpkin seeds, eggs) and vitamin A & C sources (carrots, amla, citrus fruits)


⚠ Avoid:

Scratching the scalp (worsens inflammation)

Oiling your hair too often (traps dirt & fungus)

Using hot water for hair wash


📌 When to See a Dermatologist:

If no improvement in 2–3 weeks

If scalp lesions become painful or ooze pus

If hair fall becomes excessive (more than 100 strands/day)
