My son has pfapa and epilepsy

Sia

I am really worried about my son because he has pfapa and epilepsy. He was diagnosed with pfapa at around 4 years old after having recurrent fevers, and it has been so overwhelming for our family. We thought things were under control, but a few months back he started having these weird seizures, and that’s when we figured out he also has epilepsy. It’s like a double whammy. The doctors say the pfapa and epilepsy aren’t related, but I can't help but wonder if there’s some connection, ya know? He’s on medication for the epilepsy now, but I feel like I'm lost with managing both conditions. Sometimes he has these fever episodes from the pfapa, and then I'm just left freaking out about the possibility of a seizure happening at the same time. I'm constantly watching him, the anxiety is through the roof! Is there any advice on how to cope with both pfapa and epilepsy? Are there things that we should really be looking out for or avoiding? Any tips from other parents who have dealt with pfapa and epilepsy would help a ton. I just want to make sure he’s safe and comfortable.

Doctors' responses

Managing PFAPA and epilepsy concurrently indeed requires a careful approach, especially given the overlapping concerns during fever episodes. Firstly, it’s important to stick to a consistent management routine for both disorders. For PFAPA, many find it helpful to keep a calendar tracking the timing of episodes; this can help predict recurrences and plan around them. If you haven’t already discussed other treatments with your doctor, corticosteroids like prednisone can effectively reduce fever duration in PFAPA episodes. Make sure you’re clear on when and how to use these medications as part of your routine. For epilepsy, adherence to the prescribed antiepileptic medication is vital to minimize seizure occurrences. Regular follow-ups with your neurologist is key to ensure the dose and type are optimal, especially during periods of rapid growth or other stressors like fever. Also, staying well-hydrated and maintaining a regular sleep schedule are beneficial in managing seizure triggers.

During fever episodes, introducing general comfort measures like tepid baths, light clothing, and antipyretics, such as acetaminophen, can help manage discomfort. However, ensure not to use aspirin due to the risk of Reye’s syndrome in children. Regardless of the specific condition, maintain an emergency plan for seizures, which could include having a rectal diazepam or intranasal midazolam prescription available if he experiences a prolonged seizure. Discuss with your healthcare team whether any alterations to typical medication or care are needed during fever episodes. An evaluation by an ENT specialist might be beneficial if PFAPA remains difficult to manage, as a tonsillectomy can reduce the frequency or even lead to resolution of symptoms in some cases.

Ensure you’re ready to act during fever episodes by consulting an action plan with the support team, which can include healthcare providers, school staff, and family members. Pay attention to any changes or patterns in his symptoms, as these should be communicated with his healthcare provider— this is crucial to adjust his ongoing treatment plan correctly. Joining support groups, either locally or online, can offer insights and emotional support from others coping with similar challenges, just remember each child’s situation is unique, so always balance peer advice with professional guidance. Be cautiously observant of red-flag signs such as difficulty breathing, prolonged seizures, or persistent vomiting, which may require urgent medical attention. Lastly, consider exploring stress-relief techniques for yourself, as remaining calm can significantly impact how you manage these episodes and in-turn support your son effectively.
