Am I pregnant and what are my options? - #24473

Ayra

Hello Doctor, My periods are 42 days late, which is unusual for me because my cycle is normally regular. I had unprotected sex, but my husband did not ejaculate inside. We were not sure about pregnancy, but because my periods were late, I took two home pregnancy tests on different days, and both came back positive. My husband thinks the result might be wrong, but I am worried because both tests were positive. We do not want to continue this pregnancy. I am 18 years old. Could you please confirm if I am likely pregnant and advise whether I should take abortion pills? If yes, which medication is safe and appropriate, and what precautions should I take? Thank you.

Dr. Bharat Joshi
I’m a periodontist and academician with a strong clinical and teaching background. Over the last 4 years and 8 months, I’ve been actively involved in dental education, guiding students at multiple levels including dental hygienist, BDS, and MDS programs. Currently, I serve as a Reader at MMCDSR in Ambala, Haryana—a role that allows me to merge my academic passion with hands-on experience. Clinically, I’ve been practicing dentistry for the past 12 years. From routine procedures like scaling and root planing to more advanced cases involving grafts, biopsies, and implant surgeries. Honestly, I still find joy in doing a simple RCT when it’s needed. It’s not just about the procedure but making sure the patient feels comfortable and safe. Academically, I have 26 research publications to my credit. I’m on the editorial boards of the Archives of Dental Research and Journal of Dental Research and Oral Health, and I’ve spent a lot of time reviewing manuscripts—from case reports to meta-analyses and even book reviews. I was honored to receive the “Best Editor” award by Innovative Publications, and Athena Publications recognized me as an “excellent reviewer,” which honestly came as a bit of a surprise! In 2025, I had the opportunity to present a guest lecture in Italy on traumatic oral lesions. Sharing my work and learning from peers globally has been incredibly fulfilling. Outside academics and clinics, I’ve also worked in the pharmaceutical sector as a Drug Safety Associate for about 3 years, focusing on pharmacovigilance. That role really sharpened my attention to detail and deepened my understanding of drug interactions and adverse effects. My goal is to keep learning, and give every patient and student my absolute best.

Based on the information you’ve provided, it’s quite likely you are pregnant, especially considering that you’ve taken two positive home pregnancy tests and have missed your period by 42 days. Home pregnancy tests are generally reliable when used properly, so the positive results you’ve received indicate a high likelihood of pregnancy. Even the method of withdrawal can lead to pregnancy due to pre-ejaculate fluid that can contain sperm. To confirm the pregnancy, it’s typically recommended to visit a healthcare professional for a blood test or an ultrasound. However, if your primary concern right now is not continuing the pregnancy, medical abortion can be an option. At your age and being just over six weeks delayed, medication like mifepristone followed by misoprostol within the first 10 weeks of pregnancy can be effective for a medical termination. It’s crucial to access these medications through a healthcare professional or a trusted clinic, such as Planned Parenthood, to ensure proper usage and verify that your health history supports its safe use. They will provide the medication, dosage instructions, and what to expect during the process. Be aware of the potential side effects which include cramping, bleeding, nausea, and headaches. Severe pain or prolonged bleeding are red flags that require immediate medical attention. Ensure you have a follow-up arranged after the procedure to confirm that the abortion is complete and address any complications. It’s essential to have someone with you during the process for support, and to plan some rest afterwards. Longer-term, considering reliable contraception could help prevent future unplanned pregnancies.
