What could be causing severe headaches and scalp tenderness after 3-4 years?

Hello Doctor, I have been suffering from severe headaches for the last 3–4 years, and I have had migraine symptoms for about 2 years. The pain is often very intense. I also have extreme tenderness on my scalp. If someone touches my head, applies oil, massages my scalp, or even when I comb my hair, I feel severe pain and sometimes cry out because of it. I would like to know what could be causing these symptoms and whether I need a CT scan or any other tests. Thank you.
