Kidney Transplant Patient – Need Guidance About Rising Creatinine Hello Doctors, I am a kidney transplant recipient since 2020 and currently concerned about gradual worsening of kidney function tests over the last 6 months. Current Situation Serum creatinine increased from 1.40 → 1.62 mg/dL eGFR decreased from 68 → 57 Doppler ultrasound of transplanted kidney was reported as unremarkable / normal Complete Urine test also Normal No major abnormality was found on ultrasound Current Medicines Tacrolimus total daily dose: 3 mg / 24 hours Mycophenolate mofetil total daily dose: 1000 mg / 24 hours Questions Can this gradual creatinine rise happen from tacrolimus toxicity even when Doppler ultrasound is normal? How concerning is this type of eGFR decline after transplant? In which situations is a kidney biopsy strongly recommended? Could dehydration, tacrolimus levels, chronic rejection, or medication effects cause these changes? What tests should ideally be checked before proceeding to biopsy?
