Based on the symptoms and history you’ve described, it’s important to conduct a thorough clinical evaluation to identify the underlying causes of your extreme fatigue and heart palpitations. Your symptoms could suggest anemia, possibly due to iron deficiency, given your dietary habits and the mention of heavy menstrual periods in the past. The yellowing of your eyes and dark urine could indicate issues with your liver or bilirubin metabolism, possibly related to hemolysis or other hepatic conditions. The following tests are crucial:

1. Complete Blood Count (CBC): This can help determine if you’re anemic and provide details on red and white blood cells and platelets, which can give insights into your overall blood health.

2. Serum Ferritin and Iron Studies: These are important to assess your iron stores and determine if iron deficiency is contributing to your symptoms.

3. Liver Function Tests: These can help evaluate your liver’s health and investigate the cause of the yellowing of your eyes and dark urine.

4. Comprehensive Metabolic Panel: This test will provide information about your blood sugar, electrolytes, and kidney and liver function.

5. Thyroid Function Tests: To rule out thyroid issues, which can also cause fatigue and palpitations.

6. Vitamin B12 and Folate Levels: These are essential for proper blood cell production and nervous system function, and deficiencies could contribute to some of your symptoms like fatigue.

7. Electrocardiogram (ECG): If you’re experiencing frequent palpitations, an ECG can help assess your heart’s electrical activity for irregularities.

Given your symptoms, it’s advisable to seek medical evaluation promptly, especially to rule out serious conditions like significant anemia or liver dysfunction. Alongside these tests, it would be beneficial to have a nutritional assessment to address dietary insufficiencies and deficiencies. It’s crucial, especially given your restrictive dietary practices, to ensure sufficient intake of essential nutrients. After these initial assessments, your healthcare provider can offer tailored treatment options based on the findings.
