What to do for severe nerve pain in thigh that isn't improving with treatment?
Hope you are doing well. I am Noyon, 28 years old, Male, Weight 68 kg. I am contacting you as I am suffering from severe nerve pain and need your kind advice. *Brief History:* I have been suffering from lower back pain radiating to my left/right thigh for 8 days. The pain is sharp, burning type, and increases while sleeping or resting. *Treatment Taken:* I consulted Dr. Md. Masud Rana, Assistant Professor, Medicine, MMCH on 15th June 2026. He prescribed the following medicines which I am taking regularly: 1. Mirotin 5 mg (Levocetirizine Dihydrochloride 5 mg) – 0+0+1 2. Lino M 2.5/500 mg (Linagliptin 2.5 mg + Metformin Hydrochloride 500 mg) – 1+0+1 3. Maxpro MUPS 20 mg (Esomeprazole 20 mg) – 1+0+0 4. Neucos B (Vitamin B1 + Vitamin B6 + Vitamin B12) – 1+0+1 5. Sentrx 0.5 mg (Clonazepam 0.5 mg) – 0+0+1 6. Napsod 550 mg (Naproxen Sodium 550 mg) – 1+0+1 7. Maxolax 5 mg (Bisacodyl 5 mg) – 0+0+1 8. Felbo gel - Apply 3-4 times daily *Investigations Done on 15/06/2026:* 1. HbA1c: 7.1% 2. Random Blood Sugar: 8.6 mmol/L 3. TSH: 1.59 mIU/L 4. Lipid Profile: Total Cholesterol 218, LDL 148, HDL 35, Triglyceride 173 [Borderline/High] 5. CRP: <6 mg/L 6. Xray LS Spine B/L: lumbar spondylosis *Current Complaint:* Despite taking all medicines regularly for 4 days, my thigh nerve pain is not reducing. I have severe burning/shooting pain in my thigh which is disturbing my daily work and sleep at night. Napsod and Felbo gel are giving very little relief. *Request:* Sir, I would be very grateful if you kindly review my case and advise me: 1. Should I continue the same treatment? 2. Do I need any change in medicine or dose, especially for nerve pain? 3. Should I come to Dhaka for physical examination and further tests like MRI LS Spine/Nerve conduction study? note : I took aceclofenac for reducing pain during paining, but that medicine didt not properly worked. i need a pain killer Sir.
