Investigation 1) USG abdomen + pelvis, transvaginal if possible 2) CBC 3) Thyroid profile 4) Prolactin 5) Colonoscopy

This is not a simple menstrual issue . your symptoms suggest a serious pelvic + rectal condition that needs urgent hospital evaluation or visit a nearby physician and gynaecologist. Thank you.

Severe lower abdominal and vaginal pain with heavy rectal bleeding and 4 months of missed periods sounds serious and could be due to conditions like endometriosis, pelvic infections, or bowel disease. Since pain is worsening and has been there for a year, it needs urgent evaluation with ultrasound, pelvic exam, and blood tests. Please consult a Gynecologist as soon as possible—if bleeding is heavy or pain unbearable, go to the emergency department immediately.

Based on the symptoms you’re describing, there are several potential causes that need to be assessed by a medical professional. The combination of lower abdominal pain, vaginal pain, rectal bleeding, and missed periods for four months raises the possibility of conditions such as endometriosis, pelvic inflammatory disease, fibroids, gastrointestinal issues, or even a gynecological malignancy. It’s critical to seek immediate medical evaluation due to the increasing severity of your symptoms and the rectal bleeding. Such symptoms should not be ignored. An urgent visit to a healthcare provider is necessary so that they can conduct a comprehensive evaluation. This includes a thorough history, physical examination, and likely imaging studies such as an ultrasound or CT scan. Blood tests might also be necessary to assess for anemia due to bleeding and to evaluate hormonal levels. Let’s also consider some immediate steps: avoid taking any pain medications apart from those prescribed without consulting, as they can sometimes mask symptoms which are crucial for diagnosis. Avoid strenuous activities that could potentially exacerbate bleeding or increase pain, and maintain a food log because diet could play a role, especially if the gastrointestinal tract might be involved. Meanwhile, keep track of any changes in bowel habits, pain patterns, or other symptoms and bring this detailed record to your appointment, as it can aid the medical professional in making an accurate diagnosis. Ultimately, the priority here is to identify the cause through professional evaluation and proceed with an appropriate treatment plan designed by a healthcare provider based on clinical findings. Do not delay in contacting a healthcare professional, especially considering the intensity and duration of your symptoms.
