Your symptoms are most likely due to temporary testicular congestion or mild inflammation related to masturbation, especially when it happens after a long gap (like a week). This is common and usually harmless in young men.
Why this happens
After ejaculation, especially after a gap:
Blood flow to the testicles increases The tubes (epididymis) can become temporarily congested This can cause dull pain, tenderness, or heaviness, usually on one side One testis normally hangs slightly lower than the other this is normal
This condition is sometimes called: Epididymal congestion Post-ejaculatory testicular pain Commonly known as “blue balls” (even if pain comes later)
Hi Aryan. This is a common concern, but needs attention.
Key Points:
· Possible Cause: The pain could be from muscular strain, a congested epididymis, or a slight varicocele (enlarged vein) that becomes noticeable after activity. · Concerning Sign: One testis hanging lower is often normal, but a new change in position or size needs evaluation. · Rule Out Emergency: Sudden, severe pain with nausea could be testicular torsion (twisted testis) – this is an emergency. Your pattern suggests this is unlikely.
Action Plan:
1. Immediate Rest: Avoid masturbation or strenuous activity for a few days. 2. Support & Comfort: Wear supportive underwear (briefs), apply a cold pack. 3. Monitor: If pain worsens, or you notice swelling, redness, or fever, see a doctor immediately. 4. Consult a Doctor: Schedule a visit with a urologist for a proper physical exam to rule out any underlying condition.
This is treatable. A proper diagnosis will give you peace of mind.

Hello dear I think probably it is due excess pressure on nerve during masturbation. Another chance can be forcefully ejaculation. I suggest you to please wait for 1-2 weeks. Avoid masturbation at that time for sperm recovery. In addition please get following tests done for confirmation and share result with gynaecologist or sexologist for better clarity Serum testosterone Serum androgen Esr Scrotum USG Regards
Hello
This is most likely due to temporary testicular congestion or mild irritation after masturbation, which is quite common and usually not serious.
After a gap of several days, ejaculation can cause increased blood flow and sensitivity in the testis, leading to aching pain that may come and go.
It is also normal for one testis to hang slightly lower than the other.
🛑🛑For now, rest and avoid masturbation or sexual activity for 2–3 days, wear supportive underwear, and use a warm compress for 10–15 minutes once or twice daily.
If needed, you can take paracetamol for pain. The discomfort usually settles within 24–72 hours.
Seek medical care urgently if the pain becomes sudden and severe, the testis becomes very swollen, red, or moves much higher than usual, if you develop fever or burning while urinating, or if the pain does not improve after 2–3 days.
This does not sound like something dangerous. If it keeps happening every time or doesn’t settle in 2–3 days, get a scrotal ultrasound to rule out epididymitis or varicocele.
I trust this helps Thank you!
Pain in the testis after masturbation can have several causes, but it’s often linked to minor injuries, strain, or something called “epididymal hypertension,” colloquially known as “blue balls.” This happens when arousal results in increased blood flow to the genital area without resulting in ejaculation or when there is a delay in ejaculation. Since you mentioned that one testis looks lower than the other, it’s usually normal, as it’s common for one testicle to hang lower than the other. However, if the pain persists or gets worse, it could be due to other issues like testicular torsion, where the testicle twists, cutting off its blood supply, or an infection such as orchitis or epididymitis, which might require medical treatment. It’s crucial to pay attention to any accompanying symptoms like severe pain, swelling, redness, or fever, as these signs suggest a need for immediate medical evaluation. To address the current discomfort, you could try wearing supportive underwear to reduce strain on the testes, apply ice packs wrapped in a cloth to the scrotum to decrease pain and swelling, and take an over-the-counter pain reliever like ibuprofen to help manage the pain. But do ensure you limit this to only short-term use as required. If you have any doubts or if the pain doesn’t subside in a day or two, consider making an appointment with a healthcare provider for further evaluation to rule out any serious concerns. Your testicular health is important, and persistent or severe symptoms shouldn’t be ignored.
Pain in one testis after masturbation—especially when it happens after a gap—is most commonly due to congestion (epididymal congestion/“blue balls”) or mild strain, and one testis naturally hanging lower is normal. For now, avoid masturbation for a few days, wear supportive underwear, avoid touching the area, and use warm compresses; pain should settle within 24–72 hours. Specialist consultation: see a urologist urgently if pain becomes severe, sudden, associated with swelling/redness, fever, nausea, or doesn’t improve—these are red flags that need examination to rule out torsion or infection.
