"I am a transgender woman. I have been undergoing estrogen hormone therapy, which has led to the development of my breasts. Initially, when applying pressure to the breast, there was a clear to whitish discharge. However, recently, I noticed that from one side only, pressing on the nipple results in a clear discharge mixed with blood. This symptom only appears upon expression (manual pressure) and does not happen spontaneously. I am currently living in Egypt and find it very difficult to access immediate or specialized local medical care, which is causing me a great deal of anxiety. I need medical guidance on what this could indicate and what my next steps should be."
