Dr. Arsha K Isac
I am a general dentist with 3+ years of working in real-world setups, and lemme say—every single patient teaches me something diff. It’s not just teeth honestly, it’s people… and how they feel walking into the chair. I try really hard to not make it just a “procedure thing.” I explain stuff in plain words—no confusing dental jargon, just straight talk—coz I feel like when ppl *get* what's going on, they feel safer n that makes all the difference. Worked with all ages—like, little kids who need that gentle nudge about brushing, to older folks who come in with long histories and sometimes just need someone to really sit n listen. It’s weirdly rewarding to see someone walk out lighter, not just 'coz their toothache's gone but coz they felt seen during the whole thing. A lot of ppl come in scared or just unsure, and I honestly take that seriously. I keep the vibe calm. Try to read their mood, don’t rush. I always tell myself—every smile’s got a story, even the broken ones. My thing is: comfort first, then precision. I want the outcome to last, not just look good for a week. Not tryna claim perfection or magic solutions—just consistent, clear, hands-on care where patients feel heard. I think dentistry should *fit* the person, not push them into a box. That's kinda been my philosophy from day one. And yeah, maybe sometimes I overexplain or spend a bit too long checking alignment again but hey, if it means someone eats pain-free or finally smiles wide in pics again? Worth it. Every time.

Hello

I’m sorry you’re going through this—it’s frustrating when symptoms persist for years without clear answers. Based on your description, the persistent itching and small sores on the penis since 2022 most often point to a chronic skin or infection-related condition, while the headaches and abdominal cramps are usually separate medical issues that need their own evaluation.

The common causes of long-lasting penile itching and sores include Balanitis (chronic inflammation or infection of the penile skin), Candidiasis (fungal infection causing itching and redness), Genital Herpes (recurrent small sores or blisters), or a non-infectious skin condition such as Contact Dermatitis from soaps, detergents, or friction. If the sores have stayed the same size and are only mildly itchy, fungal infection or dermatitis is statistically more likely than a serious sexually transmitted infection. Fever, however, is an important detail and should prompt proper testing.

Your abdominal cramping with constipation and gas can be related to digestive conditions such as Gastritis or Irritable Bowel Syndrome, while headaches with balance or reading difficulty may relate to Migraine, vision strain, or stress. These symptoms do not typically come from a reproductive system problem, even though they appeared around the same time.

What matters most now is getting the right tests, not just repeated medications. A clinician should examine the lesions directly and consider basic investigations such as blood sugar testing (to rule out diabetes, which can cause recurrent infections), urine analysis, and possibly a swab or blood test for infections if needed. Good hygiene is helpful—wash gently with plain water, keep the area dry, avoid harsh soaps or perfumed products, and use protection during sexual activity. A short trial of an antifungal cream such as Clotrimazole twice daily for 2–3 weeks is often reasonable if fungal infection is suspected, but persistent symptoms beyond that should be formally evaluated by a clinician, ideally a dermatologist or urologist.

Seek prompt medical care sooner if you notice any warning signs such as increasing pain, rapidly spreading sores, discharge, weight loss, severe headaches, vision changes, or persistent fever. The encouraging point is that most chronic itching and small sores in this area are treatable once the exact cause is identified.

Take care

I completed my medical degree in 2023, but honestly, my journey in healthcare started way before that. Since 2018, I’ve been actively involved in clinical practice—getting hands-on exposure across multiple departments like ENT, pediatrics, dermatology, ophthalmology, medicine, and emergency care. One of the most intense and defining phases of my training was working at a District Government Hospital for a full year during the COVID pandemic. It was chaotic, unpredictable, and exhausting—but it also grounded me in real-world medicine like no textbook ever could. Over time, I’ve worked in both OPD and IPD setups, handling everything from mild viral fevers to more stubborn, long-term conditions. These day-to-day experiences really built my base and taught me how to stay calm when things get hectic—and how to adjust fast when plans don’t go as expected. What I’ve learned most is that care isn't only about writing the right medicine. It’s about being fully there, listening properly, and making sure the person feels seen—not just treated. Alongside clinical work, I’ve also been exposed to preventive health, health education, and community outreach. These areas really matter to me because I believe real impact begins outside the hospital, with awareness and early intervention. My approach is always centered around clarity, empathy, and clinical logic—I like to make sure every patient knows exactly what’s going on and why we’re doing what we’re doing. I’ve always felt a pull towards general medicine and internal care, and honestly, I’m still learning every single day—each patient brings a new lesson. Medicine never really sits still, it keeps shifting, and I try to shift with it. Not just in terms of what I know, but also in how I listen and respond. For me, it’s always been about giving real care. Genuine, respectful, and the kind that actually helps a person heal—inside and out.

long-standing mild itching with very small sores on the penis, along with abdominal discomfort, gas/constipation, and unrelated symptoms like headache or imbalance—do not clearly point to a single serious reproductive disease, and it’s important to separate them. The genital symptoms are most commonly due to chronic mild balanitis (irritation), fungal infection, dermatitis, or less commonly a sexually transmitted infection, especially since the sores are small, stable, and not worsening; however, the abdominal pain and gas are more likely digestive issues (such as gastritis, acidity, or IBS) and not directly related to the penis. Since this has been ongoing for years without resolution, you should have a focused evaluation: a physical exam by a doctor (preferably dermatologist or urologist), and simple tests like blood sugar (to rule out diabetes), urine test, and possibly STI screening if there is any sexual risk. Avoid irritants (strong soaps, tight clothing), keep the area clean and dry, and do not self-use multiple creams randomly as that can worsen the condition.

The symptoms you’re describing - persistent itching with sores on the penis, headaches with balance issues, and abdominal pain that you suspect could be related to ulcers or gas - can be really uncomfortable and concerning. It’s crucial to your health that these are properly evaluated because they indicate underlying issues that require medical attention. Starting with the genital symptoms, the sores and itching could suggest an infection or possibly a sexually transmitted infection (STI), such as herpes or syphilis. Both can cause similar symptoms. A detailed examination by a healthcare professional, along with diagnostic tests like a swab of the sores or blood tests, can confirm this. If you’re sexually active, it’s important to get tested not only for your own health but also to prevent transmission to partners. On the other hand, the headaches and balance problems you’re experiencing might indicate a neurological issue, or they could be related to systemic conditions like an infection that also affects other parts of your body. You should see a healthcare provider who can perform a thorough neurological exam and possibly imaging tests like an MRI or CT scan. Your abdominal discomfort, resembling ulcers or gas, would benefit from an evaluation with a focus on your gastrointestinal health. Tests such as an abdominal ultrasound or endoscopy might be recommended to check for ulcers or other conditions like gastritis or irritable bowel syndrome. It’s key to connect all these symptoms as part of a bigger picture; sometimes systemic infections can cause widespread issues. If the hospitals you’ve visited so far were unable to determine the cause of your symptoms, seeking a specialist, like a dermatologist for the skin lesions or a neurologist for the balance and headache issues, may provide more insight. In the meantime, maintain good hygiene, avoid any known irritants that could exacerbate your symptoms, and keep hydration and nutrition balanced. Whenever you’re dealing with these persistent and distressing symptoms, especially when they’re impacting quality of life significantly, it’s important to be proactive and persistent in seeking comprehensive care.

I’m a periodontist and academician with a strong clinical and teaching background. Over the last 4 years and 8 months, I’ve been actively involved in dental education, guiding students at multiple levels including dental hygienist, BDS, and MDS programs. Currently, I serve as a Reader at MMCDSR in Ambala, Haryana—a role that allows me to merge my academic passion with hands-on experience. Clinically, I’ve been practicing dentistry for the past 12 years. From routine procedures like scaling and root planing to more advanced cases involving grafts, biopsies, and implant surgeries. Honestly, I still find joy in doing a simple RCT when it’s needed. It’s not just about the procedure but making sure the patient feels comfortable and safe. Academically, I have 26 research publications to my credit. I’m on the editorial boards of the Archives of Dental Research and Journal of Dental Research and Oral Health, and I’ve spent a lot of time reviewing manuscripts—from case reports to meta-analyses and even book reviews. I was honored to receive the “Best Editor” award by Innovative Publications, and Athena Publications recognized me as an “excellent reviewer,” which honestly came as a bit of a surprise! In 2025, I had the opportunity to present a guest lecture in Italy on traumatic oral lesions. Sharing my work and learning from peers globally has been incredibly fulfilling. Outside academics and clinics, I’ve also worked in the pharmaceutical sector as a Drug Safety Associate for about 3 years, focusing on pharmacovigilance. That role really sharpened my attention to detail and deepened my understanding of drug interactions and adverse effects. My goal is to keep learning, and give every patient and student my absolute best.

I am a qualified medical doctor with MBBS and DNB Diploma in Family Medicine from NBEMS, and my work has always been centered on treating patients in a complete, not just symptom based way. During my DNB training I rotated through almost every core department—Internal medicine, Pediatrics, Obstetrics & Gynecology, Surgery, Orthopedics, ENT, Dermatology, Psychiatry, Emergency medicine. That mix gave me the skill to manage acute illness, long term disease and preventive care together, something I find very important in family practice. In psychiatry I worked closely with patients who struggled with depression, anxiety, stress related problems, insomnia or substance use. I learned not just about medication but also about simple psychotherapy tools, psycho education and how to talk openly without judgement. I still use that exp in family medicine, specially when chronic disease patients also face mental health issues. My time in General surgery included assisting in minor and major procedures, managing wounds, abscess, sutures and emergencies. While I am not a surgeon, this gave me confidence to recognize surgical cases early, provide first line care and refer fast when needed, which makes a big difference in online or OPD settings. Now I work as a consultant in General medicine and Family practice, with focus on both in-person and online consultation. I treat conditions like fever, infections, gastrointestinal complaints, respiratory illness, and also manage diabetes, hypertension, thyroid disorders, and lifestyle related chronic diseases. I see women for PCOS, contraception counseling, menstrual health, and children for common pediatric issues. I also dedicate time to preventive health, lifestyle counseling and diet-sleep-exercise advice, since these small changes affect long term wellness more than we often realize. My key skills include holistic diagnosis, evidence based treatment, chronic disease management, mental health support, preventive medicine and telemedicine communiation. At the center of all this is one thing—patients should feel heard, safe, and guided with care that is both professional and personal.

Hi, I understand this has been going on for a long time and is worrying. Your symptoms are likely not all from one single disease. You have most likely separate issues, not one reproductive problem. Your Penile itching + small sores might be due to Balanitis or Candidiasis or Sexually transmitted infection, etc. Your headache+ balance issues might be due to Migraine or stress or vision problems, etc. Your Abdominal pain + gas + constipation might be due to Functional dyspepsia or Irritable Bowel syndrome, etc. Why your treatment didn’t work?- You were given antibiotics. But Fungal conditions need antifungal treatment. Functional gut issues need different management. Headache needs separate evaluation. Here is my advise-

1. For penile issue- Keep area clean and dry. Avoid harsh soaps. Physically consult a dermatologist/ family medicine specialist for visual inspection of your penile region to make a proper diagnosis and plan treatment. A chat consultation won’t help.

2. Get these tests done- CBC, RBS, HbA1C, Urine Routine Microscopy, HIV, VDRL, HbsAg, Anti-HCV.

3. For headache/balance, See a doctor urgently if Balance problem persists, Vision issues or Severe headache.

4. For stomach issues- Regular meals Avoid spicy/oily food. - Take Syrup Lactulose 15ml with warm water at night Ă— 7 days.

Seek urgent care if- Worsening balance problems, Severe headache, Large painful genital ulcers or Fever. This is not one single reproductive disease. You likely have Local genital infection, separate headache issue& digestive problem. Penile itching- likely fungal/inflammatory (treatable). Headache + balance- needs proper evaluation. Abdominal symptoms- likely functional.

Feel free to reach out again.

Regards, Dr. Nirav Jain MBBS, D.Fam.Medicine

I am a 2023 batch passout and working as a general physician right now, based in Hanumangarh, Rajasthan. Still kinda new in the bigger picture maybe, but honestly—every single day in this line teaches you more than textbooks ever could. I’ve had the chance to work under some pretty respected doctors during and after my graduation, not just for the clinical part but also to see how they handle people, real people, in pain, in panic, and sometimes just confused about their own health. General medicine covers a lot, right? Like from the smallest complaints to those random, vague symptoms that no one really understands at first—those are kinda my zone now. I don’t really rush to label things, I try to spend time actually listening. Feels weird to say it but ya, I do take that part seriously. Some patients just need someone to hear the whole story instead of jumping to prescription pads after 30 seconds. Right now, my practice includes everything from managing common infections, blood pressure issues, sugar problems to more layered cases where symptoms overlap and you gotta just... piece things together. It's not glamorous all the time, but it's real. I’ve handled a bunch of seasonal disease waves too, like dengue surges and viral fevers that hit rural belts hard—Hanumangarh doesn’t get much spotlight but there’s plenty happening out here. Also, I do rely on basics—thorough history, solid clinical exam and yeah when needed, investigations. But not over-prescribing things just cz they’re there. One thing I picked up from the senior consultants I worked with—they used to say “don’t chase labs, chase the patient’s story”... stuck with me till now. Anyway, still learning every single day tbh. But I like that. Keeps me grounded and kind of obsessed with trying to get better.

Hello Thank you for sharing your detailed history. I understand this has been a long and frustrating problem for you. Let’s break down your symptoms:

- Itching and very small sores on the front of the penis (since 2022, still present) - Headaches with imbalance, pain, and difficulty reading - Abdominal pain, possible ulcers, or a lot of gas - Persistent symptoms despite hospital visits

Here’s what could be going on:

### Possible Causes 1. Chronic genital infection or irritation: Persistent itching and sores could be due to a fungal infection (like candidiasis), a chronic bacterial infection, or even a skin condition like eczema or lichen planus. 2. Sexually transmitted infection (STI): Some STIs can cause long-term itching and sores, even if tests were negative before. 3. Systemic issue: Headaches, imbalance, and abdominal symptoms may or may not be related, but sometimes chronic infections or autoimmune conditions can cause widespread symptoms. 4. Gastrointestinal issues: Ulcer-like pain and gas could be due to gastritis, ulcers, or irritable bowel syndrome.

### What You Can Do - Genital care: Keep the area clean and dry, avoid harsh soaps, and wear loose cotton underwear. - Avoid scratching: This can worsen irritation and delay healing. - Monitor for new symptoms: Fever, spreading sores, discharge, or severe pain should prompt urgent medical review.

### What to Ask Your Doctor - Request a full evaluation by a dermatologist or urologist for the genital symptoms. - Ask for specific tests for fungal, bacterial, and viral infections (including herpes, even if sores are small). - For headaches and imbalance, a neurologist review may be helpful. - For abdominal pain, consider a gastroenterologist consult and tests for ulcers or H. pylori infection.

### Indian Home Remedies (Supportive) - Sitz bath: Soak in warm water for 10-15 minutes daily to soothe itching. - Coconut oil: Can be gently applied to the area for mild relief (if no allergy). - Avoid spicy and oily foods if you have gas or ulcer symptoms.

### When to Seek Immediate Help - If you develop severe pain, high fever, spreading redness, or difficulty urinating.

Summary:
Your symptoms are persistent and affecting multiple areas, so a thorough re-evaluation is needed. Sometimes, chronic skin/genital issues need a specialist’s eye and specific tests. Don’t lose hope—many people find answers after seeing the right specialist.

Thank you

I am currently working as a urologist and kidney transplant surgeon at Graphic Era Medical College & Hospital, Dehradun. It's a role that keeps me on my toes, honestly. I handle a pretty wide range of urology cases—stones, prostate issues, urinary tract obstructions, infections, you name it. Some are straightforward, others way more complex than you expect at first glance. Every patient walks in with a different story and that’s what keeps the work real for me. Kidney transplant surgery, though, that’s a whole different zone. You’re not just working on anatomy—you’re dealing with timelines, matching, medications, family dynamics, emotional pressure... and yeah, very precise coordination. I’m part of a team that manages the entire transplant process—from evaluation to surgery to post-op care. Not gonna lie, it’s intense. But seeing someone who’s been on dialysis for years finally get a new shot at life—there’s nothing really like that feeling. In the OR, I’m detail-focused. Outside of it, I try to stay accessible—patients don’t always need answers right away, sometimes they just need to feel heard. I believe in walking them through what’s going on rather than just giving reports and instructions. Especially in transplant cases, trust matters. And clear, honest conversation helps build that. Urology itself is such a misunderstood field sometimes. People ignore symptoms for years because it feels “awkward” or they think it’s not serious until it becomes unmanageable. I’ve had patients who came in late just because they were embarassed to talk about urine flow or testicular pain. That’s why I also try to make the space judgment-free—like whatever it is, we’ll figure it out. At the end of the day, whether I’m scrubbing in for surgery or doing OPD rounds, I just want to make sure what I do *actually* helps. That the effort’s not wasted. And yeah, some days are frustrating—some procedures don’t go clean, some recoveries take longer than they should—but I keep showing up, cause the work’s worth doing. Always is.

Hi, I’m Dr. Nikhil Chauhan (Urologist)

I’m sorry you’re going through this since 2022. Your combination of symptoms – penile itching + sores + headache with balance issues + abdominal cramping + constipation + fever – is unusual and needs urgent, thorough investigation.

Why this is concerning:

· Persistent genital sores (over 6 months) + neurological symptoms (loss of balance, headache) + abdominal pain → could point to a systemic illness, not just a local skin problem · Possibilities include: neurosyphilis, Behçet’s disease, HIV with opportunistic infection, or autoimmune vasculitis

What you must do immediately:

1. Stop relying on just prescription creams/tablets – you need a full diagnostic workup 2. Go to a larger hospital – see a Urologist + Infectious Disease specialist + Neurologist 3. Get these tests without delay: · Blood: RPR/VDRL (for syphilis), HIV, HSV serology, ANA (for autoimmune), CRP/ESR · Penile swab / biopsy (if sores persist) · Neurological exam + possible brain MRI (due to balance issues) · Abdominal ultrasound / stool examination

Do not ignore:

· If you develop severe headache, vomiting, vision change, or inability to walk – go to ER immediately

👉 You’ve suffered too long. This is NOT normal. Please see a specialist this week – your symptoms may be curable if diagnosed correctly.
