Patient Case Summary & Complete History for the Doctor:Primary Diagnosis (Endoscopy - June 11, 2026):Grade A Reflux EsophagitisErosive GastritisPast Medical, Substance & Lifestyle History:Alcohol: Consumed alcohol for 7–8 months while living in Russia. Completely sober (No alcohol) for the last 5–7 months.Cannabis/Weed: Had a habit of smoking weed, but has been completely clean (No weed) for the last 2 months.Cigarettes/Smoking: Was a regular smoker, but has completely quit smoking for the last 10 days (aligned with starting the Trio Kit).Stress & Diet: History of severe anxiety/tension and highly irregular eating habits until February 2026.Current Medications & Duration:Tab. Pantonix 40 mg (Pantoprazole): Prescribed for 2 months (60 days). Completed 34 days so far. Taken twice daily (Morning and Night) before meals.Vonocab Trio Kit: Prescribed for 14 days. Completed 12 days so far (ongoing, 2 days remaining). Hello dear I think probably it is gastric issue due to helicopter pylori or peptic ulcer Iam suggesting some medication Please follow them for atleast a week Tablet Omeprazole 20 mg once a empty stomach day for 5 days Tablet pantop D 40 mg once daily half hour before meals Light meals like dalia or khichdi Cyclopam twice a day for 5 days Avoid refined food Avoid heavy meals Yes yoghurt can be taken for gut benefits Test medication as prescribed by me can be taken for improvement In case of no improvement consult general physician medicine or gastroenterologist for better clarity Regards
