What is causing my constant overthinking and panic attacks, and how to treat it? - #29464

Client_4cfc9d

Hello doctor, I have been experiencing constant overthinking for around 4 years now, but recently it has become worse. I am a JEE aspirant and this is affecting my studies a lot. I get sudden panic attacks where I feel breathless, like I’m choking, my heart races, my body shakes, and I start crying uncontrollably. I try to control it for a few days, but then it suddenly becomes very intense. I am also not able to manage my sleep properly. Sometimes I feel very restless, and other times I end up sleeping a lot more than usual. I am also dealing with relationship issues and family stress, and I don’t really have friends or anyone I can open up to, which makes it harder to cope. I have no major past medical issues and I am not on any regular medication. Could you please guide me on what this could be, and please, suggest any safe medications or treatment that might help as it's very frequent?

I’m a periodontist and academician with a strong clinical and teaching background. Over the last 4 years and 8 months, I’ve been actively involved in dental education, guiding students at multiple levels including dental hygienist, BDS, and MDS programs. Currently, I serve as a Reader at MMCDSR in Ambala, Haryana—a role that allows me to merge my academic passion with hands-on experience. Clinically, I’ve been practicing dentistry for the past 12 years. From routine procedures like scaling and root planing to more advanced cases involving grafts, biopsies, and implant surgeries. Honestly, I still find joy in doing a simple RCT when it’s needed. It’s not just about the procedure but making sure the patient feels comfortable and safe. Academically, I have 26 research publications to my credit. I’m on the editorial boards of the Archives of Dental Research and Journal of Dental Research and Oral Health, and I’ve spent a lot of time reviewing manuscripts—from case reports to meta-analyses and even book reviews. I was honored to receive the “Best Editor” award by Innovative Publications, and Athena Publications recognized me as an “excellent reviewer,” which honestly came as a bit of a surprise! In 2025, I had the opportunity to present a guest lecture in Italy on traumatic oral lesions. Sharing my work and learning from peers globally has been incredibly fulfilling. Outside academics and clinics, I’ve also worked in the pharmaceutical sector as a Drug Safety Associate for about 3 years, focusing on pharmacovigilance. That role really sharpened my attention to detail and deepened my understanding of drug interactions and adverse effects. My goal is to keep learning, and give every patient and student my absolute best.

Overthinking and panic attacks like you’re describing can often be linked to anxiety conditions, which are notably exacerbated by high-stress situations. Your upcoming JEE exams and personal stressors could be contributing factors. Panic attacks can manifest as sudden bouts of intense fear, accompanied by physical symptoms like the ones you’re experiencing. Disrupted sleep patterns, like restless or excessive sleep, can further compound anxiety levels. Since your panic attacks and anxiety are frequent and impacting your daily life, you should consider a comprehensive management approach. Cognitive Behavioral Therapy (CBT) is a highly effective treatment option, helping modify dysfunctional thinking patterns. Reaching out to a mental health professional or counselor can be a solid first step. Additionally, lifestyle modifications may offer some support. Regular physical activity, whether it’s walking, stretching, or something more vigorous, can reduce anxiety. Ensure you’re maintaining a balanced diet and minimizing caffeine or sugar, which can aggravate symptoms. Controlled breathing or relaxation techniques practiced daily can also improve symptoms over time. Medications like SSRIs or benzodiazepines might be considered by a doctor to manage severe or persistent symptoms, but they must be prescribed and monitored by a healthcare provider. Establishing structured routines, including dedicated study times with uninterrupted breaks, can be beneficial. Since family dynamics and isolation seem to be central stressors, discussing support avenues with a therapist may provide strategies to cope better. It’s crucial to seek a timely consultation with a healthcare provider to more accurately assess your situation and formulate a detailed care plan aligned with your unique needs. If panic attacks intensify or you feel unsafe, contact a medical professional promptly, as immediate attention might be required.
