What is causing my excessive sweating and anxiety symptoms at 20 years old?

Client_46e026

Hello Doctor, I am a 20-year-old male. I would like to seek medical advice regarding excessive sweating and related symptoms that I have been experiencing for approximately 7 years. Main Complaint: I sweat significantly more than other people, even when the environment is comfortable and others are not sweating. The sweating mainly affects my face and head, but sometimes it can involve my whole body. The problem occurs even when I am alone and not physically active. I have noticed that the symptoms often become worse after masturbation. Associated Symptoms: When I am around other people or feel that someone is watching me, I become very nervous and self-conscious. My heart rate increases noticeably in these situations. My head and sometimes other parts of my body may tremble. I feel that my body movements become awkward or unnatural when people are looking at me, and I may have difficulty walking normally. These symptoms also tend to worsen after masturbation. Medical Information: Age: 20 years Height: Approximately 165 cm Weight: Usually between 55–60 kg Duration of symptoms: About 7 years Areas affected by sweating: Primarily the face and head; sometimes the entire body Current medication: Propranolol (taken occasionally for rapid heartbeat, nervousness, and related symptoms) Questions: I would like to know whether my symptoms could be related to: Hyperhidrosis (excessive sweating) Social anxiety or another anxiety disorder Thyroid dysfunction Hormonal imbalance A neurological condition Any other medical cause I would appreciate your recommendations regarding appropriate examinations, laboratory tests, diagnosis, and treatment options. Thank you for your time and assistance.

Your symptoms could indeed be related to several potential causes. Hyperhidrosis, specifically, is a condition where you sweat more than necessary, independent of temperature or activity, and it can affect various parts of the body like your face and head. Social anxiety might also play a role here—your description of nervousness, increased heart rate, and trembling when around others definitely fits. These hyper-intense reactions could intensify sweating, especially in social settings. Since you’ve noted that these symptoms worsen post-masturbation, hormonal fluctuations could potentially exacerbate the condition. For instance, adrenaline surges associated with anxiety or stress responses could be heightened after masturbation, leading to increased sweating and nervous systems activity.

To determine the underlying cause, a few paths are worth exploring. Firstly, a thorough evaluation by a healthcare provider to rule out thyroid dysfunction is important. Thyroid tests (like TSH, T3, and T4) can identify if your thyroid gland, which regulates metabolism and growth, is overactive. Additionally, considering hormonal imbalances, checking testosterone or cortisol levels could provide insight, as these hormones can influence stress responses and sweat production. It’s also helpful to consider a mental health assessment to explore the possibility of an anxiety disorder or social anxiety. Techniques like cognitive-behavioral therapy or anxiety-reducing medications might be beneficial alongside physical health checks.

For hyperhidrosis, several management strategies exist: topical antiperspirants (high-concentration aluminum chloride), oral medications like anticholinergics, Botox injections, or even iontophoresis (a dermatological treatment to reduce sweating). Each has its benefits and risks, depending on personal circumstance and triggers. Keep a detailed symptom diary noting when and how symptoms appear or escalate, which can guide treatment. Though propranolol helps manage your heart rate and could alleviate some anxiety symptoms, its use should be discussed with a doctor to ensure it’s effectively managing your situation as intended.

Ultimately, it’s crucial to consult with your healthcare provider when interpreting test results and evaluating treatment strategies. Your provider will guide you through diagnostic tests and tailor treatments to your specific case, especially as your symptoms span physical and psychological aspects. This integrative approach ensures you receive the most appropriate, targeted care.
