Hello
The thin white thread-like structure was most likely normal vaginal discharge or natural lubrication mixed with semen/pre-ejaculatory fluid. Since you did not insert the penis after removing the condom, pregnancy risk is low, though not absolutely zero because pre-ejaculate can rarely contain sperm if ejaculation happened earlier.
Mild lower tummy pain after first-time sexual activity is also quite common and can happen due to muscle strain, anxiety, deeper pelvic muscle tightening, friction, or mild irritation during intercourse.
The fact that the pain is mild, comes and goes, and sometimes disappears is reassuring.
For now, she should rest, stay hydrated, avoid repeated intercourse for a few days if uncomfortable, and monitor symptoms. If her next period comes normally, pregnancy is unlikely. If periods are delayed by more than a week, take a home pregnancy test.
Seek medical care if she develops severe abdominal pain, fever, foul-smelling discharge, heavy bleeding, burning urination, persistent vomiting, or worsening symptoms.
